  3. Providing person-centered care—Address each Elder’s clinical and health needs in the context of their personal wishes and goals.
  4. Delivering skilled nursing care—Conduct assessments, provide treatment, and evaluate outcomes specific to each Elder.
  5. Leading with purpose—As a clinical team leader, help create daily routines and experiences that reflect what matters most to each Elder.
  6. Mentoring and supporting others—Encourage growth and confidence in your fellow team members.

In fact, your skills are vital in this setting. You’ll not only provide direct care but also serve as an advocate and educator. You’ll often identify changes in health status early, guide Elders and families through treatment decisions, communicate with physicians, and ensure care aligns with each Elder’s preferences and values.

Our nurses do things a little differently than in traditional settings, but with a small number of Elders and a strong team approach, they’re not overwhelmed—and not working harder, just smarter and in more meaningful ways.

New Starting Wage :  LPN $28.33  RN: $32.    Increases for years of experience. Enrollment in LEAP apprenticeship program which provides increased wage at 6 months and 12 months as well as bonus at 12 months!
